As far as I have ever known, MOH via sip really depends on the mastered
volume for the audio source (in this case a file). Unless you are playing
through a physical gateway (FXO or PRI), there is no volume adjustment (i.e.
on a gateway you would adjust the gain, but that would also be for all
audio, not just MOH).

Does it happen locally (one handset putting another handset on hold), or
just through a gateway or sip trunk?
