As additional information, I am able to receive faxes into my asterisk based fax server and sipx now. I added a T.38 Relay profile option for DID's being used for inbound faxing to sipx.
So the question remains, even though it's working, the setup I've explained from my provider doesn't sound right to me. I still don't know how reliable that setup ends ends up being.
