Any chance you want to share who you have found/tried as an ITSP that supports T.38?
I've been a long time customer of Teliax and have found them to provide a solid service, but a bit on the pricey side. They will set up an account to handle t.38 and have been using them to terminate t.38 faxes for about a year with fairly good results. I recently started testing Flowroute and am so far very happy with them - faxing has been rock solid in the limited testing I've done so far. I've had the most trouble with faxing to toll-free carriers - could be something with Teliax. I haven't had a chance to test much on Flowroute.
