I would start by doing a pcap at your firewall to see what it shows if you want to take it up with voip.ms. I also noticed in the past couple of weeks some strange behavior, but as I am always tweaking firewall configs and testing patches its difficult for me to follow every minor issue and so I have not put resources into tracking it down.
I only started noticing issues on both inbound or outbound calls when sipx was idle from calls for a long period of time. This makes me wonder if this is something that was introduced as a change in a patch or if voip.msstarted to introduce a different keepalive method or ignores the current one, etc. If I try the call immediately afterwards, it works fine. If a sip trace doesn't show anything, the pcap from the firewall would tell volumes.
