It is really up to the admin. Rules and permissions allow one to designate who is allowed to dial certain destinations (i.e. International or orther toll calls). It also allows manipulation of the outbound number in the event different gateways or providers want a different format. The CDR also designates "long distance" in its records, etc. to help with budgeting or reporting.
