A follow-up... I don't know what happened, but suddenly everything
started working. Call transfers, conference room sound is fine, through
PSTN and remote registrations.
I literally went into VoIP.ms, disabled G.729, waited about two minutes,
confirmed the problem still existed, and then re-enabled G.729... I
tried again and now everything works. WTH???
