Todd - we have the groundwork of this done.  We use it for our click to call 
app - the first time a user logs on (assuming they are in our active 
directory), they get the below screen.  Once they key in the fields - it 
creates their user.  For our purposes, we didn't need a phone associated with 
the user (we just created a user with forwarding rules to interact with our 
legacy pbx in this example).

We did this in php with calls to the API - I'll forward the important code 
snippets when I get a few minutes.
