I think I have seen references to this a couple of times. Once where the domain file was incorrect or corrupt, the other where there was a voicemail issue (on the server).
Can you express any changes, deletes, adds, moves, restores have taken place around the time this started? Are the replicas in sysnc? If not or if you delegated a role to both servers, such as voicemail, where it is not functional might result in cases like this when it is out of sync I think.
