BW.com needs to be setup TWICE (they might send calls to you from either of their gateway). This means even if you do not have a dialplan associated with the second gateway it needs to be in the list (think of it an as ACL). So consider setting up their other IP as a duplicate gateway to receive calls in the event they do that...
Do you have your intranet subnets defined properly? Have you set your public IP address in the NAT section of the server? It is more likely you don't because sipx should be sending the public IP if it is set. Sorry I read the original post on a small device and missed your firewall config...
