I can't seem to get sipxecs to pass along a "+" as a ENUM prefix. I've entered 
"e164.org" in the base domain, and + in the add prefix, when I click Apply, OK, 
then restart the SIP Registrar and double check the setting afterwards the + is 
removed. Bandwidth.com sets this by default, as they describe in the 
provisioning email :

* Inbound and Outbound Numbers can be formatted in the following formats:

**E.164 format
Local / Long Distance calls (+1 then 10 digits)
Example: +12125551212

**10 digit dialing
Local/Long Distance calls (10 digits)
Example: 2125551212
*******************************************************************
PLEASE NOTE: E.164 is the default format; you must notify your Installation 
Specialist if you wish to use a different format.
*******************************************************************
International calls (+ country code - city code - numbers)
Example: +4402074942020
**Note: If set for 10 digit outbound then can send 011 instead of just the +

Any suggestions on that? On a related note, the default "ot.bandwidth.com" in 
the bandwidth.com template is invalid :

$ nslookup

Non-authoritative answer:

I had to use their primary and secondary IPs provided as two separate gateways, 
and also entered the port number and protocol.
