You have to run the 4.0.2 firmware, as there was a bug 4.0.1 that affected these phones working with sipxecs.
Some features will only work if you provision the phone via TFTP, and not via the GUI. Music-on-hold is one of those features. What I've done is configured the 000000000000.cfg file to look for vvx500 files in a separated vv500 directory. We have a standard config file we use, which we edit and place in that directory. We edit it for the correct mac, and add the username/password in the file. We then reboot the phone, it retrieves it's config file from that directory and MOH and MWI both work.
