replacing the phone with another model .... What do you mean???
What do you mean with 'phone'? the VoIP provider?
I'm using BroadVoice as the provider and I'm using either the UA I'm
developing or a commercial softphone (ExpressTalk).

You report that there is a phone which, during a consultative transfer,
receives a REFER request, and responds to it with a 603. That is an
error. It then (if I understand you correctly) sends the INVITE as
directed by the REFER, which is a second error.
1) What is the model of that phone?
2) If you replace that phone with a different model, do you see the
system behaving differently?
Until you get the phone(s) to behave correctly, there is no reason to
expect that you will be able to execute consultative transfers.
