I have a similar problem with my systems though they are Linux based.

With some sound devices the audio from sipxtapi to the remote device has
a low level buzz. There is also an audible low level echo. I am still
working to characterize both problems.

I suspect the echo is generated inside sipxtapi. The buzz may be device
sample rate related. I'm having a test system set up to vary the sample
rate to find out for sure. I know for certain that some devices don't
work very well at 8000 Hz sample rate - they only offer audio sampling
of 8000 Hz linear 8 bit and/or rate adjust to 11025 Hz with a very poor
quality sample rate conversion.

The problem with using a different device sample rate to internal 8000
Hz sample rate is that - on my systems at least - there are a lot of
messages about jitter buffer 'giving up' and there is a short break of
audio. This is based on 11436 revision from svn
