When it shells out after the failure, can you manually mount the
partition? Try executing the exact mount line in the script and see what
happens.

Also, scroll through the screen buffer (shift-pgup and shift-pgdown) of
systemimager during the install and see what actual command line it
tried to use, as opposed to what is in the script on the server.
