I had two things which affected rsync, symptoms appeared random. 1) iptables misconfiguration 2) dns lookups timing out (failing to contact server)
After checking the rsyncd log like Bernard suggested, check the above. I use /etc/hosts for all entries, so I remove dns from /etc/nsswitch.conf, I remove all nameservers from /etc/resolv.conf, and I remove bind from /etc/host.conf. The weirdest issue was the management node (with the image) had one particular host in /etc/resolv.conf where the administrators recently turned off bind. Dan W.
