Using Skim and vi, it works fine for me. Shift+command-click causes
Skim to pop up a vi editing the correct file.
However, you're using emacs. So, I'm guessing that there's something
wrong with the command you're using in Skim's preferences. For some
reason, it's PROBABLY just activating an existing emacs rather than
opening a new one.
I'm not an emacs user, and so I don't think I can be much help.
HOWEVER, have you read:

?? It looks like you have to put the following in your ".emacs" file (in
your home directory):
;; Starts the Emacs server
(server-start)
Additionally, you MIGHT want to also add the lines (to that same .emacs
file):
;; Auto-raise Emacs on activation
(defun raise-emacs-on-aqua()
(shell-command "osascript -e 'tell application \"Emacs\" to
activate' &"))
(add-hook 'server-switch-hook 'raise-emacs-on-aqua)
