Skim can do something that is functionally similar to this already, and it may serve your purposes. Select the text tool and highlight a section of the text. Then right click on the highlighted text and choose 'New Note or Highlight' > 'Anchored Note'. The anchored note box will appear with the highlighted text as the note title; you can enter whatever text you like as the note body and then drag the icon for the note to a convenient location on the page. You can also add your arrows or other highlights to indicate the note's target text in the page if you wish.
When you display your notes in Skim, you'll see the list of note titles which are the target text and you'll be able to jump to the pages containing the notes plus any other graphic annotations you've added. When you export the notes, you'll be able to sort them by their titles (i.e., their target text; click on the Notes column header), which should give you the groupings you want. Once nice thing about this approach is that if you have notes that target the same text across several different pages, you can group all those notes together by their titles when they're exported, but you'll also see their page locations.
