There were no larger pages.
It seems there is a coordinate system on each page that affects this. I
found that the coordinates on the first page did not match those of other
pages, so perhaps this page was somehow read by Skim (or PDFkit for that
matter) as a large page.
After printing the document as a new pdf, the coordinates were apparently
reset and the new file displays fine.

viewed side-by-side (continuous)? Some pdfs are viewed with a neat, modest
spacing, whereas others show a massive gap.
I have played around with editing the pdf, reducing the mediabox etc. (and
discovered view settings in Skim especially for that), but this did not
affect the gap. Also correcting the page size for the first page did not
help me.
