I think that's an excellent idea! I'm old ICQ user, and ICQ had some nice 
features even in the 2003b version, that's version blind ppl usually use.
If you want to play with it, you can set personal sounds for each contact 
when it comes online, when it's write to you and many more.
Besides that, ICQ has very complex privacy settings, where you can decide 
who can see you, who can't,  and eventhough I'm not using ICQ much these 
days, I miss a lots of it's setting options I believe should be in other IM 
clients.
So, for the beginning, besides this one you suggested, I vote for ability to 
set personal Skype ringtones as you can in cellphones.
