I'm a bit confused, so let me see where we're going amiss...

When you make an outgoing call, the dial pad is automatically
available when the call connects, so you can just tap digit keys to
dial digits, without doing anything else fancy to focus the dial pad
first.

When you receive an incoming call, you are right that focus will not
necessarily land on the dial pad.  Ctrl+Shift+D worked to focus the
dial pad in some cases, but it was never fully reliable.  In Skype 4.1
release though, "Dial pad" shows up in the Insert+F8 item list, and
pressing Enter there does seem to work.  An alternative to Insert+F8,
for those who prefer the Command key sequences, is Command m t.  The
Command key is the left square bracket by default.

You say you can't tab to the Dial Pad button on an incoming call.  I
am running the official Skype 4.1.0.136 release version, and "Dial
pad" is indeed in tab order, a few tab stops after the Edit box for
the call's conversation window.  I'm not sure why you wouldn't have
that button in yours.

So in summary, look for the Dial Pad button in the call's conversation
window, for both incoming and outgoing calls, and/or use Insert+F8 to
get to it if that's easier.

After placing an outgoing call, I can tab to and select the Dialpad button, 
but the same is not true for an incoming call.

Hi,
not sure if it will work, but while in a call try pressing ctrl+shift+d and
see what happens. Note that this command require you have the scripts
installed and running.
