No, never. One finger, one key.
From my experience, the ergonomic keyboard makes it simpler and more
comfortable, but I would say my hands are medium sized.

Hi!
It depends on the command. In the case of e, I use the right hand for
control alt shift. In the case of m I use the left hand for control alt
shift. I believe this would be dictated by standard touch typing
practices. Fortunately, all the keyboards I regularly use, (desktop and
netbook) have left and right complete sets of all the keys used in the
commands. If Hrvoje had used the Windows key in his command set, I'd be
in trouble for characters of the left hand.
