Hey Ken,
the only moment in which I always found the "Create account" window is after
installing Skype for the first time; if this is the case, simply close that
window with escape and insert your current login data in the Skype login
window.
If it's during download instead, try downloading Skype from this direct
download link: www.skype.com/go/getskype-full
If you're using JAWS, make sure you install the right scripts for Skype 5.x
which are actually the ones you rich at first by opening www.dlee.org/skype.
