this is great if he is using jaws and have the jaws scripts. You can use the steps up to opening the IM. when you get to that point shift tab and it takes you to the list of messages from that person. Arrow down to the message alerting you to the file then tab to the save button.

When a contact sends you a file, do the following: If you are not already in the conversation window for that particular contact, focus the conversation list with Ctrl+2, then find the name of the contact sending you the file. Now open the context menu for that contact by pressing the Applications key (or Shift+F10), and select Send IM. This will open the conversation window for that contact. Now press Ctrl+5 to get to the list of messages in that conversation. Use your up/down arrow keys to find the entry relating to the file transfer. When you find it, press Tab until you get to the Save button and press Enter or space. If you have not specified a folder for incoming files, a standard Windows "Save As" dialog box will appear. When you are satisfied that everything is as it should be, find the Save button in that dialog, or simply press Alt+S. The file will start downloading immediately. Hope this helps.
