Xproc response does not indicate errors

Trying the xproc scripting engine with an invalid xpl script produces an empty
response with a 200 ok status code.
The cause is the PipelineImpl class, which retrieves the OutputStream from the
SlingHttpServletResponse. When an error handler tries to do the same later on
to report the error, it gets an "OutputStream already retrieved" exception
