I'm not familiar with this guide, but it should be a fairly simple matter of
substituting the network interface used by a cable modem (eth0 on my
machine) with the interface used by a PPP connection (ppp0)

Take a look at the official IP-Masquerading HOWTO at
http://www.linux.org.au/LDP/HOWTO/IP-Masquerade-HOWTO.html
It covers masquerading through a dial-up link, and permanent links, and is
fairly easy to follow.

That would be the dial-on-demand, which is seperate to IP-Masquerading, but
apparently they work well together (sorry, never tried myself). It's
covered in the IP-Masq HOWTO, and you might also want to look at the diald
HOWTO (http://www.linux.org.au/LDP/HOWTO/Diald-HOWTO.html)
