If you have the kernel source (or docs) look in
/usr/src/linux/Documentation/devices.txt for the major/minor device
numbers. Then use mknod

otherwise, MAKEDEV in /dev should be able to do it for you. But I haven't
used MAKEDEV for ages...

This is more a general /dev question, it isn't really specific to SCSI.
