Have you considered checking the FreeBSD 2.x bugs lists to see if its
addressed in there?
The combination is sound - I use the duo together both at home (on
Linux, NetBSD, Irix, etc) and at work (on Linux and Solaris).

There is a FreeBSD users group in Sydney - admitantly, my only
experience with them was being flamed by one of their zealots for
posting an apology to a mispost. You may have better luck asking them
since their list memebers are more likely to have been using a similar
combo.
And I wasn't being hostile. If I was, you would have known it.
Other thoughts are: make sure your ssh is generating wtmp/utmp
entries, since sudo uses these.
