What kernel version?

This sounds suspiciously like what my box was doing under a bodgey 2.4.4
kernel when it kept giving kernel paging faults - the superficial
processes kept running - but nothing new could be spawned by init or by
anything else forking or whatever.

Hard reboot was the only way to get it back.

Check your logs, and look for "Unable to handle kernel paging request"
errors - sometimes they made it into the log on my box.
