What sort of barcode scanner type are you using?
1) a contact wand you drag over the barcode
2) a CCD you hold very close to or on the barcode
3) a linear laser
4) a multi-angle laser
5) a digital camera

Just use the isolation principle to help diagnose where the fault is.
Try other barcodes
Try other scanners with your barcodes

If the problem is with your barcodes try looking at them and decoding on 
paper whilst refering to the Code39 chart... an excercise for the reader 
is to find the chart. Then look at what's missing.

Problems can be with contrast and alignment. These tend to be less of a 
problem with bigger 'characters' IE try printing 50mm high, 150mm wide vs 
5mm high, 50mm wide. Oh yeah.. if the ink's fading, try running through a 
photocopier with high contrast as a last ditch effort (maybe enlarge too).

At the office we've got thermal transfer and also laser printers printing 
code39 barcodes. All our scanners are keyboard wedge devices which makes 
things easy.
