We were doing this to the UK for a few months. Fat pipe here, cable connection there. Worked very well, but we're using a big, over-priced box with an alcatel label on it and IP phones at either end. The bandwidth is probably there, the latency would be the killer. play with gnomemeeting to get a feel for this stuff. The technology is already there and it works pretty well.
