The above two points seem crucial for 'timely' (i.e. fast) ssh logins.
In the past, my ISP has misconfigured the reverse lookup for my IP, and ssh to certain sites would take yonks (over 2 minutes!). I would eventually get in, but only after sshd seemed to wait for DNS timeouts or something similar.
