It's a bit customized to my particular products so you'll need to tweak it more. I'll try to remove as much garbage as possible before sending it.

I'm using (actually just stopped using) Lotus Database. If the templates are compatible, I can give you my template. (I stopped using it because it was too time consuming for what I need to do and my sales/marketing person has her own program to keep track of customers.)
