Just had a forehead-slapping moment noting that the size was set, but there was no image_uuid at all in the JSON. So that sort of explains the no-disks problem (why didn't it just fail or give me an empty disk, though?).
So I added that, removed the size field and tried again with the JSON below. That gets me several minutes in the "provisioning" state before transitioning to "failed".
