For a long time the SmartOS USB images were built using a prebuilt USB
image rather than making one from scratch.
I did contribute a patch ages ago that is able to build USB images from
scratch:
https://github.com/joyent/smartos-live/commit/579a0cce5a44006146af792ffd8db78cdaf56d1c
If that code isn't currently being used to generate the USB images,
perhaps it should be.

This happened on two different physical machines, with different USB
Sticks, freshly written or updated platform dir, and confirmed
correct downloads. Booting with a ISO image on the other hand does
not show this problems.
Any idea what could be causing this?
