I have verified that USB legacy booting is enabled in the BIOS (two independent 
systems, one HP workstation and one SuperMicro server).

Using 
https://us-east.manta.joyent.com/Joyent_Dev/public/SmartOS/smartos-latest-USB.img.bz2

Using the Windows tools, I have done this in diskpart first:

list disk
select disk X
clean
exit

Second, writen (unzipped) .img using Win32DiskImager

Now back in diskpart. Notice the partition is not active by default, but even 
doing this manually (as in the Mac instructions) doesn’t help:

I had similar issues, make sure when they system boots it is using the USB 
image and not UEFI. Follow the USB instruction for windows or Linux and you 
should be good. Just remember don't boot UEFI.
