With option 2 do you really need separate threads? In my system I have the main thread in which starts the service and periodically queries the DB for outgoing messages and queues any messages found to be sent. Messages are read using the inbound message notification callbacks and written to the DB. There are other threads that SMSLib creates under the hood but I only deal with one thread - no synchronization issues.
