What do you use instead of iCal?

Set an alarm on that.

Or set up a google account, use the calendar, set it up for email and/or
text reminders at user defined times.  And you can subscribe to them in
iCal, or even set it up as an exchange account.

Failing that, get in the habit of starting iCal when you turn your mac on :)
