Phil Software is Arts PDF Variform www.artspdf.com. I remember I bought the Plus version at a previous company when I needed to do something similar but couldn't get it to work and used the Lite version.
I was originally using a Department of Health issued PDF which I then added the form fields to in Acrobat but made a number of subsequent forms in various applications and added form fields to them.
