Are you using any software to implement GTD or just using a workflow on your Mac?
I have implemented GTD for several years now using OmniFocus ( http://www.omnigroup.com/products/omnifocus/ ) and what you are talking about I do by processing my Sent folder each day and copying anything I'm waiting for a reply from into OmniFocus and then sets its status appropriatly. I'm sure other software such as Things ( http://culturedcode.com/things/ ) would offer the same or any of the exploding number of task managers and GTD apps. Also, MailTags ( http://www.indev.ca/MailTags.html ) may have something to help if you are looking for a more generic solution though I'm not sure about that as I've not considered it for your specific requirement.
