Hello Andrew,

Open your multi-page PDF document with Preview.
Select View > Thumbnails

To delete a page, select its thumbnail and hit backspace
To reorder a page, drag its thumbnail into the new position


To insert a page (or more) from another PDF:

Open both PDF documents with Preview.
Select View > Thumbnails in each

Drag the thumbnail of the required page from one document ON TOP of a thumbnail 
in the destination document - the thumbnail border goes dark grey. Don't drag 
the thumbnail above or below as then you will just get one window showing two 
documents but they won't be merged.

Save your documents.

Annotations can be added by clicking the small pen in the box icon on the 
toolbar and choosing the required tools.
