Have you got the ipad on "only update when connected to wifi"?

I've noticed my ipad sometimes doesn't update photo stream until I open the
photos. Not consistent though.

between two iCloud accounts then you have to individually save each photo
into that shared photostream on the phone, there's no automatic way but
perhaps that'll be sorted in iOS7?
