I can't speak for other apps, but on Scan that I have, the screen displays a 
set of "sights" when launched. Point the camera at the QR code or barcode until 
it's within the sights and at that point my app flashes and if it's connected 
to the internet takes be to the web page for the QR code.

Something like that!
