Try logging into iCloud.com and add a Reminder there, see which device it doesn’t sync down to.
On any offending devices, go to Settings > AppleID > iCloud > Reminders > switch it off & on again. Ideally select to delete Reminders already on the device (if there aren’t any important ones which haven’t synced up to iCloud yet) > Switch it back on.
