I use Advanced Circuits at <www.4pcb.com> . They are located in Colorado. I get their $33 special which is any size up to 60 sq-in with solder screen and solder mask on 1 or both sides , plated thru holes on 2 sided FR4 for $33 each. The only downside the special has a min. quantity of 3. I fit as many designs as I can in 60 sq-in then cut them apart with an 18" paper cutter. They also have a free DFM service that you upload your Gerber files to and they E- Mail you a report of any errors or manufacturing issues. They do offer 4 layer, but not in the $33 special.
