May be caused by bad reception.
Can you check your signal quality ?

Are you using both (tv-out + vga-out) output connectors at the same time ?
With epia I use only vga-out. So I disable tv-out at module load time.

In one of your other logs, I saw the line:

[setup-softdevice] deinterlace method set to 1 lavc

I don't know if that setting has some influence, when using hw-decoder.
I guess it should be ignored in that case, but it is not :-( .
So disable postprocessing !
