This might be a dumb question, but how does one "vote" on a package?

Sometimes a comment box shows up on entries, and sometimes it does not.

Also, there needs to be a clearer status message at the *top* of the 
review page or somewhere prominent that indicates whether a package 
really *can* be reviewed, or whether it's reached a final status.

Right now this is a fairly confusing interface.
