Thanks for the response Jan. We had moved from 10g to 11g before doing the upgrade which seemed to be the root of the problem. We did get some errors when upgrading schema from 1.1->1.2, but those were thought to be resolved from a fix given in a mailing list thread. Maybe this is where the issue started.

In the lookup_checksum procedure, it is being populated from database sequence rhnChecksum_seq. So it looks like your rhnChecksum_seq is giving out lower values than there are already stored in the rhnChecksum table. Can you run select max(id) from rhnChecksum; select rhnChecksum_seq.nextval from dual; Did you perhaps drop and recreate that sequence?
