I've also seen ALL_TRUSTED being hit even when it's clear that
the mail has passed through relays that are indeed untrusted.
Mostly I see this in conjunction with the following debug output,
where none of the Received lines seem to be recognised by SA:
debug: metadata: X-Spam-Relays-Trusted:
debug: metadata: X-Spam-Relays-Untrusted:
...and if there are no relays that are untrusted, then ALL_TRUSTED
ends up being set.
An example of a message coming in with Received lines which cause
ALL_TRUSTED to be set when it shouldn't is here:
