Compile the regular sendmail with milter support. Use a milter
that can send the mails to another host for checking (e.g.
spamass-milter can be patched to do this, I think).

Or switch to postfix or exim, as others suggest.

One annoying thing with running Interscan as gateway to the
normal sendmail (it forwards mails to the normal sendmail on
port 10025) is that it won't preserve the ip of the host that
sent the mail to the server - it'll be replaced by 127.0.0.1
as that's where the normal sendmail gets the mail. Does
some harm to the effectiveness of rbl tests. The "sendmail
edition" of Interscan (a milter) might be interesting.
