OK, you've got SA installed. Have you installed the prequistes (Net::DNS, berkely DB modules)?

If you're using MailScanner, then you'll have to turn spamassassin on in the MailScanner.conf file.

As to why is doesn't do all this automatically - well there are a thousand ways of doing email on *nix, and it would be very hard to cover all the options. So you do it manually.
